Social Worker
Summary:
The Dialysis Social Worker works with people affected by emotional, mental, behavioral, and medical issues to help them cope with the challenges in their lives in relation to their treatment needs at the renal center.
Qualifications:
Education:
Master’s Degree in Social Work from a college or university that is accredited by the Council on Social Work Education (CSWE)
Licenses and Certification:
• Master’s Degree in Social Work with a specialization in clinical practice from a school of social work accredited by the Council on Social Work Education.
• Current CPR certification through American Heart Association.
Experience:
• Minimum of one year’s professional work experience as a Master’s CSWE certified work as a Social Worker.
• Prefer at least one (1) year experience working with renal dialysis.
• Basic computer skills and proficiency in MS Word, Excel, PowerPoint, and Outlook required.
Essential Functions:
• Provides psycho-social support to patients in the dialysis and home dialysis settings which includes providing timely and appropriate assessment, interpretation and diagnosis of mental, social, and emotional disorders in collaboration with other professionals.
• Performs initial assessments and then yearly assessments as well as assessments for possible changes in patient’s condition. Completes progress notes at least quarterly.
• Completes annual review of patient’s life plan or long-term care services to make sure that their current needs are understood and met.
• Assists patients in understanding and coping with their medical diagnoses and situations which may require additional education and patient/family reflection of that education.
• Counsels patients about their illness and advises family care givers.
• Provides treatment interventions that include, but are not limited to, individual, group, child, and family therapy, counseling, behavior modification techniques, and coping strategies.
• Interviews patients, assesses needs, and assists in developing a treatment plan and coping strategy.
• Provides information to other health care professionals about the psycho-social issues that may impact the patient's treatment.
• Assists with making the necessary adjustments to the patient's lifestyle and ensuring the necessary support system is in place.
• Provides information and refers patients to appropriate services including support groups, home healthcare providers, assisted living facilities, grief counselors, and legal aid.
• Assists with scheduling appointments for or with patient.
• Acts as a liaison between clients, family members, and the health care system
• Assists patients with health care insurance paperwork, financial assistance programs, and benefits claims.
• Monitors adherence and need for adjustments to treatment plan and recommendations.
• Maintains current and accurate records in compliance with policies, regulations, and legislation.
• Acts as Grievance Officer for the dialysis center for concerns and their follow up.
• Acts as a resource or preceptor for teammates with less experience.
• Acts as a resource or subject matter expert for other facility personnel.
• Verifies accurate charges and documentation are submitted in a timely manner.
• Complies with dialysis facility policies and procedures for infection control, safety, and quality.
• Responds appropriately and aids in emergency situations. Remains on-site as necessary staff for evacuation needs of any patients as set forth by Program Manager and Dialysis RN Manager.
• Provides crisis management leadership and support as needed.
• Regularly communicates with Program Manager to plan, confirm, coordinate, and organize patient care.
• Assists the Program Manager in quality measures, i.e., audits, infection control compliance, accreditation preparation, etc.
• Fosters Quality Improvement by participating in Quality Meetings. Track and trend concerns and other Social Service flagging areas to present at QAPI meetings.
• Participates in dialysis stand-up meetings, team meetings, and phone/web-based conferences as required.
• Informs Program Manager and Medical Director of all incidents, conditions, problems, counseling regarding teammates, patient care, or facilities.
• Other duties and responsibilities as assigned.
Knowledge, Skills, and Abilities:
• Possesses working knowledge of assessing and addressing the specific needs of the targeted population.
• Possesses sound working knowledge of different treatment modalities.
• Demonstrates familiarity with all aspects of psychopathology.
• Demonstrates proficiency in treating different developmental stages.
• Demonstrates knowledge of the Hemodialysis and Peritoneal Dialysis Process.
• Demonstrates ability to assist with providing quality outcomes and tracking of metrics.
